How To File A Claim
To quickly process your claim please have the following prepared before calling:
  1. The manufacturer of your phone.
  2. The model name or model number of your phone.
  3. The ESN, IMEI, MSN or MEID of your phone, this is typically located underneath the battery and is 8 - 15 digits long. If your phone was stolen, check the packaging that your phone came in, receipt or call your carrier.
  4. The person filing the claim must be listed on the account.
For Stolen Phones:
  1. Call your wireless service provider to suspend your service to protect yourself against unauthorized charges.
  2. You may be required to report the incident to the local police department. If a police report is required, you will need the following information:
    The name of the Police Department where the incident was reported.
    Police report case number.
    Police report case number If the police department does not complete a police report, you must provide us with the name of the Police Department, the phone number, and the person you spoke with so that we may verify the information.
For Lost Phones:
  1. Call your wireless service provider to suspend your service to protect yourself against unauthorized charges.
